The Disease Management (DM) program is an additional program for people with a chronic condition such as Diabetes, Asthma or High Blood Pressure. The DM Nurse team can assist by:

• Providing education on your condition and on the specific questions you may have on managing your condition.
• Offering 1:1 coaching via phone to help you reach your health goals
• Providing information to help you make informed health care decisions

With the DM program you have access to:

• A 24 hour nurse line available 24/7
• A Health Information Library with hundreds of prerecorded topics.
